Abstract

Patient safety is defined as the avoidance of unintended or unexpected harm to people during the provision of health care. Hospitalized patients are at risk of suffering an adverse events and patient on medications have the risk of medication error. The magnitude of the problem of unsafe care became a hot issue after the publication of the report of institute of medicine in USA, entitled to error is Human (1999) and the second report by the United Kingdom Department of health, an organization with memory (2000). Both reports scoped the subject of safety and harm in health care and provided the first estimate of the burden of patient harm. Since that time, patient safety became a new health priority and new field of research in health services. Health care professionals (HCP) must be familiar with patient safety concepts and principles. Spread of the culture of patient safety among HCP is a crucial step which will lead to improvement of quality and safety of health care.
Patient safety is the business of everyone, and ranges from patient to politician.
